US Script On-line Applications

Note: The following applications require the new JInitator be installed on your system. Please read and follow the instructions below.

• Member Eligibility Application
• Workers Comp Application

We have recently upgraded our server. In order to use this application, you must have the latest JInitiator plug-in installed.

Installation requires that you use the Local or Domain Administrator account to ensure that permissions are set correctly. Please contact your local Windows administrator if you do not have the required privileges.

Please download the JInitiator to your desktop. Double click on the Jinitator file you downloaded and click on "Run" and accept the defaults.

Next, you need to install this security file.

After you have downloaded the file to your desktop, double click it to run the installer. Click on Unzip to install the file. Click the close button. Note that this step also requires Administrative Privileges.

Now you MUST reboot your computer.

You should now be able to log in to our application.
